REQUEST FOR AUTHORIZATION FOR THE COLLECTION OF PERSONAL AND HEALTH DATA
VitalFit PROGRAM FOR PRESCRIPTION OF A PERSONALIZED PHYSICAL EXERCISE PLAN
The purpose of this document is to provide you with information on the procedure for collecting some personal and health data necessary for the design of a personalised physical exercise prescription plan, as well as to inform you of the previous physical tests to be carried out, in order to request your authorisation to do so.
Before signing your authorization, you should read this document and ask any questions you need to understand the details about it. The decision is voluntary and may be revoked by you at any time during the development of this program.
The results of the medical and physical tests may be used to carry out epidemiological studies to assess the number of users of the programme, the differences in performance by sex and/or age groups and the degree of improvement in the physical condition of those participants who join the plan. The values of these results will be analysed anonymously, i.e. without any personal data that would allow identifying who they belong to. They can be presented and/or published in scientific journals, conferences in any format (paper, poster, communication) or teaching activities.
Personal, biometric and medical data: Name and surname, sex, age, telephone number, email, weight, height, blood pressure, heart rate, injuries and/or diseases of interest, anthropometry.
Physical tests that can be performed (1, 2): Balance, walking speed, strength and/or mobility of upper and lower limbs, strength and/or mobility of the trunk, resistance to effort.
Only duly authorized personnel of our entity will be able to become aware of the information we request from you. Likewise, those entities that need to have access to it so that we can provide you with our services, or those entities to which we are obliged to provide your personal data due to compliance with any law, may become aware of your information. There are no plans to transfer your personal information outside the European Economic Area.
We will retain your data for the duration of our relationship and for as long as we are required to do so by law. Once the applicable legal deadlines have expired, we will proceed to securely delete them.
